    Responsibilities

    The employee in this position serves as a Program Support Assistant to the Virtual Health Program supporting the Home Telehealth program including the Lead Care Coordinator and other Care Coordinators. Duties are varied, non-routine, and consist of handling data and coordination for program patients, registration for My HealtheVet, managing clinics for real time Clinical Video TeleHealth events, including patient education activities, technical and scheduling activities, and other program support duties, as assigned. Position entails travel between the East Orange and Lyons campuses as well as to any of the ten VA NJHCS Community Based Outpatient Clinics. Main Duties and Responsibilities:
    • Assists in enrollment, disenrollment and troubleshooting of telehealth equipment;
    • Receives and processes documents and reports in accordance with established procedures;
    • Organizes and maintains patient data and retrieves a variety of computerized data;
    • Collects, compiles and tracks statistical information and prepares reports;
    • Keeps Home Telehealth Lead apprised of patient census and any changes/problems;
    • Establishing and maintaining inventory of all telehealth equipment;
    • Monitors the use of services, supplies, and equipment for the program;
    • Operates web-based software for Telehealth program as well as other databases for VISN
    • Maintains calendar of patient appointments;
    • Coordinates meetings, conferences and takes meeting minutes;
    • Prepares travel documents;
    • Performs other related duties as assigned.


    Work Schedule: Full Time, Monday thru Friday; 8:00 am - 4:30 pm.
