The VA Greater Los Angeles Healthcare System (VAGLAHS) is one of the largest, most complex health care systems within The Department of Veterans Affairs. It is one component of the VA Desert Healthcare network (VISN22) offering services to veterans residing in Southern California and Nevada. VAGLAHS Consists of three ambulatory care centers, a tertiary care facility and 10 community based outpatient clinics. VAGLAHS serves veterans residing throughout five counties. There are 1.4 million veterans in the VAGLAHS service area. VAGLAHS is a teaching hospital, providing a full range of patient care services, with state-of-the-art technology as well as education and research programs. VAGLAHS includes inpatient facilities at the West Los Angeles campus, two community living centers at the West VAGLAHS has the largest Community Care Program in the nation that supports homeless Veterans. VAGLAHS Community Care Homeless Program, which includes facilities and staff dedicated to the support of veterans who are homeless, is the largest in the nation. Duties
The Senior social worker demonstrates possession of advanced practice skills and good clinical judgment. The Senior social worker in this position will provide services in the Homeless Patient Aligned Care Team clinic in Community Engagement and Reintegration (CERS) where the clinic is located at the West Los Angeles Healthcare Center, a co-located integrated homeless PACT (HPACT). The HPACT clinic team will strive to improve continuity of care and coordination of services for Veterans who are either homeless or at risk for homelessness by working with existing clinical and community care programs. The Senior social worker will demonstrate an ability to connect and coordinate multiple care and service needs, using effective interactional skills in collaborating with staff that currently provide services for homeless Veterans. Offering specific services in this manner will provide an opportunity for the social worker to demonstrate improvement in patient centered care for homeless Veterans.
Tour of Duty: The HPACT social worker will provide services 5 days a week and 40 hours a week. The regular tour of duty for social workers is 8:00am to 4:30pm, however, this position includes providing social work services during the evenings up to 9:00pm or 10:00pm and on weekends. Services could be expanded to 10:00 pm and the tour of duty will be adjusted to accommodate the evenings and weekends.
